Mother of all coincidences. Did they ever rerelease it with a changed cover or did they scrap the whole thing?

Ramones - Leave Home w/ Carbona
Gwar - Carnival of Chaos promo CD with the Star Trek samples that were removed from the standard release
Dead Kennedys - Fresh Fruit for Rotten Vegetables on Cherry Red with the original, uncensored back cover
Guns N Roses - Appetite w/ the original cover

Not me, but my workplace as one of the handful of DualDisc copies of Time Out by Dave Brubeck that actually hit the street (under 50 known to exist?).

I almost bought one of these. It was on eBay for about three months and was only $120 IIRC. I passed because I figured somebody else would get more enjoyment out of it. Seemed to take forever to sell.

I have this. I was lucky to get one from a BMG rep before she had to send them back to corporate to be destroyed.

i bought the 12 inch, and didn't know it was withdrawn,

i did wonder why there was no cd-single for it though.

i also have a withdrawn 7 inch and 12 inch from frazier chorus - heaven.
again, i got both of them much later used. not sure if there was some controversy about
    the release.

They changed the cover and put their "Majesty" log in place of the flaming apple.

Not only did I get this one, I bought three more for my brother in law and some friends when I heard they were thinking of pulling it.

I have a copy of Kylie Minogue - Impossible Princess which between the issue of the promo and it's eventual eponymous titled release Princess Diana died in the "accident" on the Paris underpass.

The 2007 Sundazed CD reissues of the first three Moby Grape albums, pulled because Matthew Katz still owned the rights to the band name, I believe.

I also have the briefly legitimate Little Feat, “Late Night Truck Stop” 2CD but that’s easily available in other forms.

EMI's 2003 reissue of The Yardbirds "Little Games" with 2 BBC tracks that (at the time) had been unissued - "White Summer" and "Dazed and Confused". Luckily I had this on preorder at Amazon.uk - it wasn't until quite a while later that I read it had been recalled.

Run2 I believe was over that John Denver lawsuit.
Frazier Chorus, good question. The God Like Instrumental from the Ray 2x12 was as far as I went with that tune.

How about any Van Morrison album later than whichever the most recent is? Does that count? :(

I don't think the LP was ever withdrawn, was it? I know the song wasn't on the cassette or CD, but I am pretty sure the LP always had it.
