Starting off with "Oh My Darling, Clementine".

Clementine falls into the river and drowns while her boyfriend is unable to save her, due to not being able to swim.

What else? Please post examples and lyrics and/or descriptions of the death if possible.

This thread might be more fun if it was about where the girl gets some.

Nick Cave and Kylie Minogue: 'Where the wild roses grow'

As Cave, as killer, is singing in that song 'all beauty must die'. And so does Elisa Day.
The song is from Cave's album 'Murder Ballads', so many more characters will find there, euh,... a tragic end.

'Mom and Dad' by the Mothers of Invention. An eerie presage of Kent State and CSNY's 'Ohio'

Your child was killed in the park today
Shot by the cops as she quietly lay.
By the side of the creeps she knew
    They killed her too.

True, but the first song listed by the OP was "My Darling Clementine," hardly a Black Sabbath song! :)

If been doing folk for almost 30 years. Its almost like every other song. Even going back to the broadsides and olde ballads. Banks Of The Ohio was the first song I learned on the guitar.
