How about the poster included with Van Halen's Women and Children First that features a picture of David Lee Roth chained to a fence? :D

This classic one from Imagine has already been mentioned:

I always liked this one from Rush - Hemispheres:

This one isn't really a fold-out poster, but its a nice insert photo of J.R.R. Tolkien from Bo Hansson's LP of moog music inspired by Lord Of The Rings:

This poster is now hanging on my 8 year old daughter's wall. I asked her who her favorite is and she pointed to Ace. Parenting win.

Chicago at Carnegie Hall is my favorite. It towers over all others. Does anyone know if this is the largest poster ever included with an LP?

Album poster, (but this image is cropped way in tight and color is poor).

Top left: R. Lamm, keys -- W. Parazaider, reeds -- D. Seraphine, drums
Mid left: T. Kath, guitars -- P. Cetera, bass
Front left: L. Loughnane, trumpet -- J. Pankow, trombone
