1960s-1970s TV shows whose intros changed more than twice

    Seasons 1-2: "Friends Are There"
    Seasons 3-5: "We're Ready to Party!" (replaced the original intro for the season 1-2 episodes in syndication)
    Season 6
    Season 7 (though on DVD, the Season 6 intro is used instead)

    For syndication, the 73 episodes (not exactly making up the entire first four seasons, as some were omitted to include a few season 5 episodes) used the end titles from seasons 5-6.

    The soap opera The Doctors had at least 2 opening titles/logos.

    The first one is highlighted by Young and the Restless-influenced head drawings of each of the cast principals that would be arranged in a circle. The second one is the logo, but has Elizabeth Hubbard sitting at her desk.

    "CHiPs" had at least 3 opening titles and 2 different versions of its main title music; the season 6 main title theme was just a shortened version of season 2-5's with the first few seconds removed.

    Season 1 (1977-1978)

    (original John Parker version of main title theme)

    Seasons 2-5 (1978-1982)
    Chips Season 2 Opening and Closing Credits and Theme Song
    (main title theme music re-scored by Alan Silvestri)

    Season 6 (1982-1983)
    CHiPs Intro - HD - Season 6
    (shorter version of the season 2 theme)

    Alan Silvestri did re-score the main title theme for seasons 3 (1979-1980) and 4 (1980-1981) as well, but these were never actually used on the series.
