I remember the late Ian McLagan telling me that he did the cover when I brought my copy for him to autograph (yes, the version of the album with the stitch-sewn cover). the polaroid mosaic is by the late village artist Jimmy DeSana. he's credited as the artist, not just the photographer, but Byrne is credited with the concept. it lives at the rock hall museum. (I'm glad you mentioned mind games--I really love that sleeve, both sides.)
c'mon, blah blah pages and what no X...!, !?... I'm pretty sure these 3 at least were by Exene. I always wondered where she got wrapping paper with skull and cross bones, number 13 etc. after seeing the Wild Gift cover.
Chris Dreja of the Yardbirds (who did their Roger the Engineer) also did the back cover band member photos for Led Zep I.
There are a lot of Art School dropouts-turned musicians out there, e.g., Jagger, Bowie, Lennon. Debora Iyall did most of Romeo Void's. Somebody beat me to Exene's covers for X. I believe Stipey was always originator (if not final executor) of R.E.M.'s cover art, though they paid a lot of money to Anton Corbijn, too.
