I never knew of that one.
I was working at HMV (in the UK) when 9/11 happened and the day before there was a DVD release of the kids cartoon series "Danger Mouse" which was titled "Tower of Terror". The front sleeve depicted a building exploding. We were asked by head office to remove all copies from sale and replacement sleeves with new artwork were sent out.

My first UK copy of Whammy! just had a sticker with Moon 83 on it to be stuck over Don't Worry on the cover. Obviously cheaper than junking all the covers. For some reason though it was stuck randomly on the front cover rather than the back.

These are the ones I have off the top of my head:

Kevin Rowland ~ My Beauty (with Thunder Road)
Thompson Twins - Roll Over UK 12"
Bronski Beat - Run From Love UK 7" & UK 12"
Bronski Beat - This Heart UK 12"
Frankie Goes To Hollywood - Liverpool (2CD reissue with tracks later removed)
Simple Minds - The Early Years CD
Sparks - I Want To Hold Your Hand UK 7"
    New Order - Run 2
Robin Scott - Woman From The Warm Grass (first withdrawn CD issue without bonus track)
The B-52’s Love Shack ’98 CD Single (later issued as Love Shack ’99)
XTC - Wrapped In Grey CD Single - cost me 50p!
    B-52’s - Whammy! (with Don’t Worry)
B-52’s - Mesopotamia (with original long mixes)
Sigue Sigue Sputnik - 21st Century Boy (7” single, original withdrawn mix)
The Revillos - Attack! album. The band were unhappy with these demos being issued as an album. Apparently they destroyed any copies they found for sale. It's not that rare though. The CD versions (on Receiver as Attack Of The Giant Revillos and Captain Oi!) both use different mixes to each other and neither have the vinyl mixes on.
The Human League - Holiday ‘80EP (12” reissue planned for 1982)
BA Robertson - Platinum Collection CD
Heaven 17 - At The Height Of The Fighting (He-La-Hu) (7" only commercially issued in the UK on 12")

Love this thread, want to bump it up.

I haven't got any that were inmediately withdrawn, but I've got the original CD of the Bambi Molesters' debut album Dumb Loud Hollow Twang. Dirty Old Town apparently went to loan sharks to get it released in Croatia, it got licensed and released in similarly dismal numbers in Germany, and went out of print within a year. There may have been a surf music boom in Europe in 1997 but evidently not enough to keep this album in print. It did get a vinyl reissue at some point, but the CD wasn't, and is still incredibly rare. I bought mine from a guy in Germany who I believe worked sound for the band around that time. Fans of surf music, check it out. The remake of the album in 2003 includes, as a bonus track, "Cecilia Ann" written by one Steve Hoffman. Why does that name seem familiar?

But anyway. That doesn't really count. Would love to read more!

Finally have a real one - We Are Jonah by the Waterboys. Two individual CD singles released at the same time, and according to Wikipedia, were immediately recalled. I've been looking since May for these... I wasn't happy about the price tag, but I wasn't going to wait even longer and pay even more...

The original Dedicated promo CD copy of Spiritualized's Ladies And Gentlemen We Are Floating In Space with the "Elvis" version of the title track. Quickly pulled at the behest of the Presley estate (but showed up years later on the deluxe reissue).

The original version of the "Grace of My Heart" soundtrack that has Joni Mitchell singing a slow, piano version of "Man from Mars". It was quickly replaced with a version with a different singer.

One I don't have, but have heard recently ripped from 8-track - Country Life by Delaney and Bonnie from 1972. Jerry Wexler at Atlantic didn't like it and sold their contract to Columbia. Actually a good record.

That album has some amazing Bruford work. Cheesy as heck looking artwork though and the sound isn’t stellar, but, some real great performances that I don’t believe are even in the boxed set (I might be wrong about that).
