Let's add the French "Martyrs" to the list...the film manages to be 1. Brutal 2. Somehow Plausible. 3. Mournful. It's a combination that is difficult to shake.
Not necessarily saying it was the best movie in the world but there was a movie released in 2008 called Untraceable which featured someone torturing his victims live on the internet - the torture would increase based on number of hits on the website. It always stayed with me because the obvious message was the idea that society itself was the monster!
The two that very effectively gave me chills in the last 20 years or so were the Ring and The Grudge. The first that ever scared me was watching the Screaming Skull. I was about 5 years old and it was on late at night at my Grandparents and the final scenes scared the crap out of me. Of course I had to get a VHF tape of that movie 50 years later just because.
When I was young (before I became an atheist) I used to think The Exorcist was because I believed that could actually happen to someone. Now I don't really know. To be honest I don't watch a lot of horror movies, but I can't think of any that actually scared me.
I don’t find torture porn to be scary just repulsive. Films that involve supernatural horror are always fun even though I don’t believe in the supernatural.

Great one! I love Ezra Miller! I'm partial with films he's in, but it was a really good movie regardless.
I was going through a string of comedies, some light fare, and saw this at the library and decided to check it out. Whoah, what an intense film. yeah, Miller was great in the role.
What's the name of that movie where they are looking for, or discovering, an old haunted building, kind of like a documentary? I think 90's, perhaps? Anyway, I started watching it one night on TV, missed the beginning, but, it was really scary. The only really, truly scary movie i had ever seen. Most of the others that peeple say are scary, are not, really. Well, it turns out that the film I'm talking about, was a hoax. And maybe most already knew that it was, but, on that night, watching it, i believed it was a real movie, made by kids, who were trying to document these ghosts. 2nd place goes to Wait Until Dark, Audrey Hepbern (?) , 1960's, as a little boy watching it at a drive-in movie.

I only thought the end was kind of scary when one of the guys was just standing in the corner. Don’t know exactly why I found that disturbing, but that was about it. Most of what preceded that was mostly annoying and I thought over-acted.
Aside from getting stomach heaves and almost having to leave the theatre because of the spinning cameras, yeah it was pretty tense.
