In the book Tom thinks to himself how lousy the food is ...and it does look kinda crappy in the movie..

 

Not quite sure what the movie conversation is when Michael and Tom discuss how Barzini will try to eliminate Michael after Vito dies but the book is something like

Tom “Do you know how they will come after you?”
Michael: “Pop told me. He said they would you some one close to me that I would never suspect. It will be either Carlo, Clemenza or Tessio. They won’t use Rocco Lampone because he is not close enough. They won’t use Al Neri because he used to be a cop. They won’t use you because you are Irish”
Tom: “I’m German-American “
Michael:”To them that is Irish”.

In the book Tessio calls Michael on the telephone with the meeting ; he doesn’t approach him at the cemetery

 

Sollozzo: 'What's done is done. Now don't lose that famous temper of yours, eh Sonny?'

Also on the book front: does the book explain how they got the head in the bed? Seems to me that’d be tough to do without waking Woltz up.

 

I don't believe it does....and maybe it's just as well.
It would have been a considerable undertaking.
Whoever did it ( Luca Brasi ? ) would have to go to the stable , hack the horse's head off then break into the house , find the bedroom, take the bedsheets off, put the head in the bed and then make good their escape without any of the staff knowing it.
